North Cape Center is pleased to announce that in the Spring of 2012, they will be branded as a Cardiac Management Program.
Cardiac Management at Genesis is designed to meet the individual medical care and sub-acute rehabilitation needs of patients with a variety of cardiac diagnoses such as: post cardiothoracic surgery, post cardiac stent, observation post pacemaker, coronary artery disease, congestive heart failure, implanted cardioverter/defibrillator, life vest monitoring, post ablation and lvad recipients.
Our Cardiac Services were developed in collaboration with local cardiologists, referring physicians and local hospitals.
Some of our in house services will include: cardiac rounds with a consulting cardiologist, clinical staff specially trained in cardiac care, ongoing interdisciplinary clinical team meetings, lab testing and EKG/Xray available 24/7, Specialized cardiac diets and nutrition management, respiratory therapy services, follow-up phone calls after discharge, collaboration with hospital, outpatient rehab program and specialty rehabilitation services in cardiac care.
According to MedPAC, five of the top seven conditions that constitute the most spending on 15 day readmissions are cardiovascular in nature, and heart failure tops with chart with a 12.5 percent 15 day readmission rate.
With this in mind, North Cape Center will help patients with congestive heart failure actively manage their disease along with the physician to insure a successful and sustainable return to their daily routine.
Based on patient-specific evaluations and tests, North Cape will develop treatment plans which may include: a diet to prevent excess fluid accumulation, daily weight monitoring, fluid management, regular monitoring of blood pressure and pulse, formal rehabilitation therapy designed to address your specific needs, assistance beginning a smoking cessation program, lifestyle changes and stress management, strategies to promote increased activities, medication and medication management to help the heart work better and manage symptoms and education is key to the self-care for a successful discharge.
